​Patients of Central KW Family Health can click on Patient Portal to be able to do the following tasks:

 

​1. Submit forms to be completed by your physician (fee) ​​​

2. Request an insurance note for massage therapy, physiotherapy or orthotics (fee)​

3. Update your personal information including health card, address or email

​

4. Provide vaccination updates that you have received in the community

​

5. To submit vaccine forms to be completed for school/work (fee)

​

6. Request a transfer of your records if you have moved/found a different doctor (fee)
